Program Guidelines
This plan cannot be combined with other dental plans or insurance. All plans are non-refundable. If you choose not to use your benefits, the annual fee is still non-refundable. Payment for care experiences is due at the time of scheduling. Missed hygiene reservations or cancellations without at least 2 business days’ notice will count as one of your included visits.
How to Enroll
Signing up is simple. Just speak with one of our front office Care Creators and we’ll take care of the rest. Your membership begins the day you sign up and is valid for 12 months. No membership card needed— your plan details are kept securely on file.

This Plan Cannot Be Used:
Alongside any other dental insurance or discount plans
For services or injuries covered by workers’ compensation or auto insurance
For treatments that fall outside the scope of our team’s expertise (as determined by your dentist)
For referrals to outside specialists
For hospital visits or hospital-related charges
